Linkage to Community Services from Prison

Professional Case Management provides linkage to community services to inmates (both male and female) from prison. We will help to prepare and integrate the prison inmate into the community by either providing one-on-one case management or referring the inmate to various re-entry programs, or a combination of both. PCM has experience in working with the prison population and is aware of many of the challenges that emerge during the re-entry process. PCM can help persons returning to the community with appropriate referrals to:

  1. Clinical programs, such as inpatient hospitalization, outpatient treatment, partial hospital or day treatment programs.
  2. Employment opportunities. Job counselors or job coaches.
  3. Housing programs, Section 8, H.U.D. subsidized and public housing.
  4. Crisis Services usually 24 hour/7 days per week availability.
Referrals can be made by inmates themselves, family members or from the Department of Correction (DOC).

Note: It is always helpful to PCM to have as much advance notice as possible when a person is going to be released from a correctional facility. This becomes especially useful in order to facilitate good treatment and housing options.

A variety of General case management interventions can be provided to persons who are returning to the community from the prison population. This can range from general, on-going, personalized case management, to specific needs such as providing help looking for appropriate housing, clinical services or employment.
