Geriatric Case Management Services


Professional Case Management or PCM can provide case management to the elderly to accomplish specific goals or for on-going support.

Home Visits can be arranged on a schedule that suits you best. They can be conducted either in your home, facility or in a mutually agreed upon public place, such as a library or coffee shop.

Relocation and help with making difficult decisions around moving can be provided by PCM. You, or your loved one, may need help in processing the required decisions toward considering moving to an assisted living facility or nursing home. You may need help in deciding about moving in with friends or relatives.

Linkages to medical services are a primary case management function and may include finding necessary medical services and providers. Perhaps you need help in finding a particular specialist or a specific service such as a home health aide, a live-in companion or a visiting nurse.

Professional Case Management or PCM can provide case management to the elderly to accomplish specific goals or for on-going support. Housing Advocacy is also a key function of PCM. If you're on a waiting list for elderly housing but don't seem to be getting anywhere, PCM can help.

Prescription drug assistance programs, such as Conn-Pace where the qualified applicant pays a fee per prescription and the State pays the rest, are available and we can assist you with these processes. Many medications have drug assistance programs offered through the pharmaceutical company, which usually include a long and complicated application process.

Establishing trust funds, living wills and estate planning is a common concern for the elderly. PCM can provide help and support, through a referral process, to qualified attorneys who are experienced and reliable.

Budgeting and paying bills is a service that can be provided, either through PCM or through an appointed conservator of estate. This can often be a stressful task and unnecessary stress can be reduced or eliminated by this intervention.
When a judge of probate appoints a conservator of person, estate or both person and estate, there are many decisions and actions that can be addressed successfully. PCM can help determine the need for a conservator and help facilitate the process.

Family Meetings can be arranged through the intervention of PCM that can serve to improve the communication that is vital to helping an elderly family member.

Senior Centers

Many towns in Connecticut have their own senior centers that provide a variety of services for seniors 55 and older. Some senior centers provide meals one or two times per week as well as occasional evening activities.  There are many kinds of formal and informal programs, ranging from singing, dancing, arts and crafts, movies or day trips. Transportation is often provided to those who don’t drive through dial-a-ride or other town transportation services. There is usually a nominal annual fee for  transportation services.  Some companies require advance notice in order to plan their pick-up and delivery schedule.

While some individuals are naturally gregarious, others are private people who don’t easily join group activities. Most of the senior centers that we have encountered are quite responsive to the needs of every individual. PCM recommends family members and friends become involved not only in the referral process but by participating regularly with their loved one in a chosen activity. We believe that this kind of commitment can make all the difference in the successful integration process.
