Consultative Case Management Services
If you are an attorney
visiting this site, you may want to consider hiring Professional
Case Management (PCM) to assist with your client's case. Under this
arrangement, we will collaborate with your client to provide the best
possible outcome, both legally and clinically. As the representative
counsel, you will be in total control of what is recommended and what
is presented to the Court. We will be able to provide letters to the
Court, on-going reports and the like, which will all be funneled through
you for your final approval. In addition, we will be building a relationship
with your client, establishing a rapport and creating a solid foundation
for clinical stability and a positive outcome. There may be cases which
involve several layers of case management services. If you are aware
of specific issues or situations that need addressing, we will be pleased
to provide the service that is requested.
We will need to have a contract for fees for service. In the section under "fees", I have described PCM's hourly rate and will require payment as the services occur. If you wish to retain Professional Case Management as an on-going consultant, we can tailor an agreement that allows for the varieties of service delivery, the level of support required and the longevity of the case. In addition to Court Supportive Services, PCM can also provide other services such as linkage to probate court needs, geriatric case management, general case management services or juvenile cases.
If you are a Clinical Professional such as a PhD; MD; LCSW; APRN or a psychotherapist, you may either want to refer your client to PCM either directly or through your agency or organization. We will collaborate together on the treatment plan and come up with the best possible plan for the client. If your referral is court related, we will provide the on-site support required in order to produce a positive outcome. If PCM is working as a consultant for you or your organization, we will need to establish a contract for fee for service. PCM requires payment for services rendered at the time of service delivery. If the projected longevity of the case is difficult to determine, a retainer fee can be arranged to cover a period of time in advance for services provided. Rates may vary, depending upon the level of support required and the longevity of service.
