Geriatric Case
Management
Professional Case Management provides geriatric
case management, either for specific goals or for on-going support.
A list of services includes:
- Home Visits
- Help with processing decisions to move to a different location, e.g.; nursing home placement, assisted living, move in with friends or relatives.
- Linkage to medical services, e.g.; visiting nurses, home health aides, and finding needed medical services.
- Housing Advocacy-help with waiting lists, etc.
- Help with Conn-Pace applications and other prescription
drug assistance programs
- Referrals for establishing trust funds, living wills, estate planning.
- Help with budgeting, paying bills.
- Help in finding or petitioning for a conservator of person or estate.
- Coordination of family meetings.
